In any emergency it is vital the we as emergency medical providers get as much information on our patient as possible.  In many cases this can be difficult if the patient is incapacitated and there is no one there to provide us vital information like medications, or past medical history.  In an effort to improve this situation the public safety committee has come up with a simple information sheet for people to use in their homes.  Simply print out the following form, fill it out and post it on your refrigerator.  This will make it much easier for us to do our job and provide the care that you need.
